Hospice and Palliative Care

No one wants to think about losing someone they love. For that reason, many families delay discussions about end-of-life care. That is why it should be brought up before you need Hospice or Palliative Care.  Our experience at Fairmount shows that the earlier a resident is approved for Hospice the better their care is and the more comfortable they are.  Gathering information about when to call hospice before a crisis hits will give you time to understand the different options available and to determine the best fit for your loved one.  Fairmount and your doctor will be right there helping you choose the best time and the preferred Hospice.

Hospice care begins after a resident has been assessed and approved by Medicare while in residence at Fairmount.  Many families wait until the last 1-2 weeks of life. This means that they have missed a great deal of care that could have been provided several months in advance. The hospice team knows the dying process that most of us don’t really understand.  They can provide residents and their families a wealth of education, emotional support and hands on attention that is necessary for people at this stage of life.  Many families find renewed closeness during the time they are receiving hospice care which greatly improves the quality of life for our resident. This is much more difficult when the hospice team has only a few weeks to manage care and offer support to the family.

A resident can graduate out of Hospice and this happens more than most people realize.  My own father graduated from Hospice and lived many years after.   

Hospice care is an additional layer of support for your loved one and works in partnership with your family, your physician and the Fairmount staff. When a patient begins hospice care, they receive a team of support for their physical, emotional, and spiritual needs. In addition to this team of care, all medication related to the patient’s illness and all medical equipment and supplies related to this diagnosis are provided at no cost to the patient or family.

Bristol Hospice
Our Bristol Hospice programs provide hospice services to meet the physical, psychosocial, and spiritual needs of our patients and their families/caregivers. An interdisciplinary group of professionals and volunteers develops an individualized plan of care which includes, as appropriate, the following services: nursing, physicians, home health aides, counseling, spiritual support, therapy, dietary, volunteers, durable medical equipment, supplies, and bereavement services.
