Fairmount Assisted Living & Memory Care

Comfort-Focused Care

Hospice care

When comfort matters most, we surround your loved one — and your family — with compassion, gentle attention, and unwavering support.

HOSPICE PARTNERSHIP

Familiar faces, familiar home

When a family chooses hospice, they shouldn't have to choose a new place to call home. We coordinate closely with the family's chosen hospice providers so your loved one can remain right here, in the surroundings they know.

Hospice nurses and aides work hand in hand with the Fairmount staff who already know your loved one's routines, preferences, and personality. That continuity means fewer strangers, more comfort, and care that feels like it comes from people who genuinely care — because it does.

YOUR CHOICE OF PROVIDER

We work with the hospice you choose

Fairmount is not a hospice agency, and we don't ask families to use a particular one. Hospice is your decision — we're the home your loved one stays in while receiving it.

Our staff handle the everyday: meals, dressing, bathing, medication reminders, and the ordinary companionship of the house. The hospice team brings their nursing visits, equipment, and clinical direction. We keep the two in step so nothing falls between them, and so your family isn't the one carrying messages back and forth.

If you're not sure where to start, our team can talk you through the local providers families here have worked with — and what to ask them.

HOSPICE PARTNERS

Partners families here have worked with

We don’t require a particular hospice — the choice is always yours. But families often ask which local providers we’ve worked alongside, so here are two teams we know and coordinate with closely.

Catalina Hospice

A locally owned, patient- and family-centered hospice, with more than 120 years of combined hospice experience across their team.

Harmony Hospice

Focused on compassionate, loving care and enriching education for patients, families, and the wider community through every stage of the journey.

A peaceful common space for family visits

WHOLE-FAMILY SUPPORT

Support for the whole family

This season is tender for everyone who loves your parent, not only for your parent. We hold space for the whole family — offering a listening ear, practical guidance, and emotional support whenever you need it.

For those who find comfort in faith, a visiting chaplain can offer spiritual care and prayer. And our own staff, who often grow close to residents and families alike, are here to walk beside you with warmth and compassion every step of the way.
